CARVER CENTER SUMMER PLAYGROUND 2011
Carver Summer Playground is 5 fun filled weeks of
well supervised active games, arts/crafts, board games, bowling, swimming, and much more.
Get together with old friends and make some new friends.

* Medical Insurance provider and policy number *
* Date of last tetanus immunization *
